Objectives and competences

Student will: ? understand chronic diseases in an individual, ? understand the importance of empowerment and health literacy for managing chronic diseases, ? Understand nurses' role and functions in preventing and managing chronic diseases.

Content (Syllabus outline)

Chronic diseases and their impact on the individual. Risk factors for chronic diseases. Self-management of chronic diseases. Empowerment of patients with chronic disease The importance of health literacy in managing chronic disease. Special features of nursing care for the patient with chronic disease. Integrated care for patients with chronic disease Programs of prevention of chronic diseases and their activities. Outpatient clinics and their role in the prevention and monitoring of chronic diseases. Health education centers and their role in the prevention of chronic diseases.

Intended learning outcomes - knowledge and understanding

Upon competion of the course, student will be able to: • explain the impact of chronic disease on an individual and the risk factors related to chronic diseases, • explain the importance of health education for lifestyle change and the importance of empowerment for self-management of chronic diseases, • describe the importance of health literacy for self-management of chronic disease, • describes nurses’ tasks in the outpatient clinic of family medicine and health education center.

Intended learning outcomes - transferable/key skills and other attributes

Students apply the acquired knowledge to nursing practice.
